73歲的李爺爺身體相當硬朗，每天有8、9個小時都在打理農務，然而雙膝內側軟骨因長期磨損，雙腳中間呈現可放入一顆籃球大小的O型腿，使得行走不便且疼痛難耐，經醫師評估其雙腳膝蓋變形程度超過45度，診斷為嚴重的退化性關節炎。在接受兩側人工膝關節置換手術後，恢復良好已能正常行走，且大幅改善膝蓋關節變形、施力困難及關節磨損造成的疼痛問題。
人工膝關節置換手術治療
大千綜合醫院骨科醫師容志雄表示，目前人工膝關節置換手術有一般傳統手術及微創手術等方式，一般醫師會建議患者施行成功率最高、風險較低的單側人工膝關節置換手術解決關節問題，手術時間約需1小時左右，術中出血量小於50C.C，相對其他手術安全許多，術後能順利解決關節磨損造成的疼痛，及關節變形難以施力的問題。但部分患者因關節退化程度嚴重，若分期開刀患者的預後通常較不理想，而不得不改採風險較高的兩側人工膝關節置換手術。
同時施行兩側人工膝關節置換手術
由於李爺爺骨骼較大，又為嚴重兩側膝蓋退化性關節炎，容志雄醫師考量兩腳膝蓋若分開手術，在兩次手術中間，完成手術的單側膝蓋雖恢復正常曲線，但未手術的一側仍彎曲45度，在過渡期內雙腳會呈現一高一低的情況，使行走較術前更加困難，也影響術後的肌力恢復緩慢，因而予以施行兩側人工膝關節置換手術，李爺爺預後情況較單側良好，也能免去經歷辛苦、漫長的復健過程。
有效解決患者病痛
容志雄醫師指出，雙膝關節置換術雖然手術時間、出血量及風險都較單側手術大幅增加，而臨床上也有許多醫師顧慮其手術風險，而不敢輕易施行。但其實只要在術前謹慎評估，降低手術風險並與病患詳細說明與溝通，這樣的手術能更有效的成功解決患者病痛。手術後患者配合醫療團隊及復健團隊規劃的復健療程，運用助行器、專用膝關節訓練機器來幫助矯正姿勢及訓練肌肉力量，在術後的一週就能康復出院。
